ข้อ ๔ รูปถ่ายที่ติดบัตรประจำตัวพนักงานเจ้าหน้าที่ให้ใช้รูปถ่ายที่ถ่ายไม่เกินหกเดือน ก่อนวันยื่นคำขอมีบัตรประจำตัวพนักงานเจ้าหน้าที่ ขนาด ๒.๕x ๓.๐ เซนติเมตร ครึ่งตัว หน้าตรง ไม่สวมหมวกและแว่นตาสีเข้ม แต่งเครื่องแบบปกติขาวหรือเครื่องแบบปฏิบัติราชการ
