ข้อ ๔ ผู้ใดประสงค์จะมีไว้ในครอบครองหรือใช้เครื่องกำเนิดรังสี ให้ยื่นคำขอรับใบอนุญาต ต่อเลขาธิการ พร้อมด้วยเอกสารหรือหลักฐานตามที่ระบุไว้ในแบบคำขอรับใบอนุญาต
