ในกรณีที่ใบอนุญาตหรือใบรับแจ้งสูญหาย ถูกทำลาย หรือชำรุดในสาระสำคัญ ให้ผู้รับใบอนุญาตยื่นคำขอรับใบแทนใบอนุญาตหรือใบแทนใบรับแจ้งภายในสิบห้าวันนับแต่วันที่ได้ทราบถึง การสูญหาย ถูกทำลาย หรือชำรุดในสาระสำคัญ การขอรับและการออกใบแทนใบอนุญาตหรือใบแทนใบรับแจ้งตามวรรคหนึ่ง ให้เป็นไป ตามหลักเกณฑ์ วิธีการ และเงื่อนไขที่กำหนดในกฎกระทรวง
