Section ๑๐

ข้อ ๑๐ ในกรณีที่ใบอนุญาตเป็นผู้ประกอบกิจการตามมาตรา ๕ สูญหาย ชำรุด หรือถูกทำลายในสาระสำคัญ ให้ผู้รับใบอนุญาตยื่นคำขอรับใบแทนใบอนุญาตต่อเลขาธิการ ตามแบบ กฮ.๓ ท้ายกฎกระทรวงนี้ การออกใบแทนใบอนุญาตตามวรรคหนึ่ง ให้เลขาธิการออกใบแทนใบอนุญาตตามแบบ ใบอนุญาตเดิมโดยมีคำว่า “ใบแทน” ด้วยหมึกสีแดงกำกับไว้ที่ด้านบนของใบอนุญาตนั้นด้วย
