ในกรณีที่ใบอนุญาตสูญหาย ถูกทำลาย หรือชำรุดในสาระสำคัญ ให้ผู้ได้รับ ใบอนุญาตยื่นคำขอรับใบแทนใบอนุญาตต่ออธิบดีภายในสิบห้าวันนับแต่วันที่ได้รับทราบถึงเหตุดังกล่าว การขอรับใบแทนและการออกใบแทนใบอนุญาต ให้เป็นไปตามหลักเกณฑ์ วิธีการและเงื่อนไข ที่กำหนดในกฎกระทรวง
