เลม่ ๑๓๘ ตอนที่ ๑๓ ก หนา้ ๗ ราชกิจจานุเบกษา ๑๕ กุมภาพันธ์ ๒๕๖๔ กฎกระทรวง การจดแจ้งและการออกใบรับจดแจ้งผลิตหรือนำเข้าเครื่องมือแพทย์ พ.ศ. ๒๕๖๓ อาศัยอำนาจตามความในมาตรา ๕ วรรคหนึ่ง แห่งพระราชบัญญัติเครื่องมือแพทย์ พ.ศ. ๒๕๕๑ และมาตรา ๑๙ วรรคสอง แห่งพระราชบัญญัติเครื่องมือแพทย์ พ.ศ. ๒๕๕๑ ซึ่งแก้ไขเพิ่มเติมโดยพระราชบัญญัติเครื่องมือแพทย์ (ฉบับที่ ๒) พ.ศ. ๒๕๖๒ รัฐมนตรีว่าการ กระทรวงสาธารณสุขออกกฎกระทรวงไว้ ดังต่อไปนี้
