ข้อ ๔ ผู้แจ้งต้องจัดทำรายงานแสดงปริมาณการครอบครองวัสดุนิวเคลียร์ต่อเลขาธิการ ทุกรอบหนึ่งปีนับแต่วันที่เลขาธิการออกใบรับแจ้ง และให้จัดส่งรายงานพร้อมด้วยเอกสาร หรือหลักฐาน ตามที่ระบุไว้ในแบบรายงานภายในสามสิบวันนับแต่วันที่ครบกำหนดระยะเวลาจัดทำรายงานดังกล่าว
